With our onsite clinic, we take over as a participating senior’s primary care provider. Our dedicated physicians, nurse practitioners, and specialists see seniors for routine appointments and monitors changes in their health while seniors enjoy other services. Medical services include: 
• Primary, nursing and specialty care

• Vision, dental and hearing care 
• Foot care 
• Mental health services 
• Personal response systems 
• Medical equipment and supplies 
• Hospitalizations and nursing home care if it becomes necessary

Trinity Health PACE Montgomery County rehabilitation therapies provide the tools and techniques needed to heal and regain strength. We offer physical, occupational and speech therapies to support each participant on their path to recovery. Therapists specializing in senior care work closely with you and your care team to improve functionality, ultimately promoting independence for seniors.

Because many seniors experience difficulties in getting to and from doctor appointments, our wheelchair-friendly vans provide transportation and access to care. We offer free transportation to and from the Trinity Health PACE Montgomery County center and specialty medical appointments. Our trained drivers accommodate each participant, ensuring their comfort and safety.

Trinity Health PACE Montgomery County offers recreational therapy activities and hobbies to improve physical and cognitive skills, as well as create opportunities to socialize. Our recreational therapists plan engaging, fun activities for participants to enjoy in the center. Some activities include:

• Bingo and trivia 
• Day trips 
• Holiday and cultural celebrations

Trinity Health PACE Montgomery County Social Workers coordinate care and resources to support seniors and their families. Positioned as the senior’s advocate, our social workers help to provide services including counseling, caregiver education, and access to financial or legal guidance resources. We connect you to the resources you need, so you and your family can make the best decisions possible. You can feel at ease knowing you are not alone as you navigate your journey to wellness.

Proper nourishment is a key component of your comprehensive care plan. Trinity Health PACE Montgomery County provides Registered Dietitians to assess each senior's nutritional needs. This includes factors such as risk for disease, any health challenges and accessibility to food. A nutritious breakfast and lunch are provided at the center and home-delivered meals can be arranged, as necessary.

When daily activities become burdensome, our in-home support can help. Our personal care aides will visit the home as needed and help participants complete tasks like bathing and dressing, meal preparation, laundry and light housekeeping.

Trinity Health PACE Montgomery County believes spiritual care plays an important role in healing the mind, body, and soul of our participants. Encouraging participants to seek meaning, hope and spiritual connection, chaplains and spiritual companions offer emotional support and resources to participants and family members alike.

Specialty medical care including dental, podiatry, optometry, and audiology services and supplies is available by our network of medical specialists. Referrals for specialty care must be ap­proved by the Medical Director and interdisciplinary team as being medically necessary.

There are no out-of-pocket charges if participants qualify for both Medicare and Medicaid or Medicaid only. For participants who don’t qualify for Medicaid, but have Medicare, there is a monthly fee. For participants who do not qualify for Medicaid or Medicare, there is a monthly fee.
