Trinity Health PACE Montgomery County is Now Open!

As the first Trinity Health PACE program in the state of Maryland, we are thrilled to serve seniors in the Montgomery County area. PACE stands for Program of All-Inclusive Care for the Elderly, and our compassionate, comprehensive care allows seniors to age in place, helping them maintain independence as long as possible. Trinity Health PACE programs are nationally recognized for providing high-quality, holistic care for seniors, and as one of the largest PACE providers in the country, we proudly care for over 3,000 participants.

Located in Gaithersburg near Montgomery County Agricultural Fair Grounds, our center of 17,576 square feet offers medical and social services for participants and features three activity rooms, in-house laboratories, a large rehabilitation therapy space, and five exam rooms. The facility is close to many scenic areas for day trips including the Seneca Creek State Park. We're equipped to enroll up to 200 Montgomery County seniors.

Trinity Health PACE Montgomery County’s community-based care gives seniors the support they need to live on their own or with family. Our unique care model partners with both caregivers and participants, offering a full spectrum of services including:
