Mercy LIFE West Philadelphia provides an experienced team of healthcare professionals who work closely with each participant to evaluate their individual needs and develop a customized program of care. Our Day Center offers a convenient location where an interdisciplinary care team of doctors, nurses, therapists, and social workers can provide preventive healthcare as well as observe and respond to changes in each senior’s health. All medical care is provided by Mercy LIFE West Philadelphia primary care providers (PCPs), nurse practitioners and registered nurses. Medical services include:
• Primary, nursing, and specialty care
• Medications
• Vision, dental and hearing care
• Foot care
• Mental health services
• Personal response systems
• Medical equipment and supplies
• Hospitalizations and nursing home care if it becomes necessary

Mercy LIFE West Philadelphia provides rehabilitation therapies designed to reduce pain, increase mobility and regain functionality. Our skilled therapists specializing in senior care provide physical, occupational and speech therapies designed to rebuild strength and promote independence for seniors.

Free transportation is provided to and from the Mercy LIFE West Philadelphia center and specialty medical appointments scheduled by LIFE. Our trained drivers offer the accommodations and accessibility needed so seniors experience safety and comfort.

Staying active is important for mental and physical health. Mercy LIFE West Philadelphia provides Certified Recreational Therapists to create engaging and fun activities for participants to enjoy in the center. Different hobbies and exercises increase wellness and create an environment to socialize and mingle. Some activities include: 
• Arts and crafts 
• Chair yoga or aerobics 
• Computer Classes 
• Bingo and trivia 
• Day trips 
• Holiday and cultural celebrations

Serving as your advocate, Mercy LIFE West Philadelphia’s social workers provide guidance for each participant and their families. Whether it is counseling services, caregiver education or access to financial or legal resources, our social workers collaborate with other members of your care team to promote physical and mental health. We help seniors and their loved ones navigate important decisions, aiming to make a positive impact on their overall wellness.

Registered Dietitians determine each senior's dietary needs to make sure they receive adequate nutrients and nourishment. Based on their individual health needs, the dietician will develop a healthy and balanced meal plan. A nutritious breakfast and lunch are provided at the LIFE center and home-delivered meals can be arranged, as necessary.

When regular tasks become difficult, our in-home support services are here to help. Personal care aides visit the home as needed, helping seniors complete activities like bathing and dressing, meal preparation, laundry and light housekeeping.

With a full-circle view of each participant, we understand the significance of treating the whole person – in mind, body and spirit. Mercy LIFE West Philadelphia chaplains and spiritual companions play an important role in healing and seek to provide peace and encouragement. Our compassionate staff creates opportunities for seniors to find meaning, hope and spiritual connection.

Specialty medical care including dental, podiatry, optometry, and audiology services and supplies is available by our network of medical specialists. Referrals for specialty care must be ap­proved by the Medical Director and interdisciplinary team as being medically necessary.

There are no out-of-pocket charges if participants qualify for both Medicare and Medicaid or Medicaid only. For participants who don’t qualify for Medicaid, but have Medicare, there is a monthly fee. For participants who do not qualify for Medicaid or Medicare, there is a monthly fee.
