LIFE St. Mary understands the challenges of aging and staying independent. LIFE St. Mary’s team of skilled professionals support the health and independence of seniors so they can continue living safely at home in Bucks and Montgomery County, Pennsylvania. 

Services Include:

All medical care is provided by LIFE St. Mary primary care providers (PCPs), nurse practitioners and registered nurses. Medical Services include: 
• Primary care 
• Nursing care 
• Specialty care 
• Medications 
• Vision and hearing care 
• Dental care 
• Foot care 
• Mental health services 
• Personal response systems 
• Medical equipment and supplies 
• Hospitalizations and nursing home care if it becomes necessary 

Therapists specializing in senior care help LIFE St. Mary participants maintain mobility and function. Physical, occupational and speech therapies provide techniques and equipment that promote independence for seniors. 

Free transportation is provided to and from the LIFE center and to specialty medical appointments. 

Staying active is important for mental and physical health. LIFE St. Mary certified recreational therapists create engaging and fun activities for participants to enjoy in the center. Some activities include: 
• Arts and crafts 
• Computer Classes 
• Gentle movement 
• Pet therapy 
• Bingo and trivia 
• Holiday and cultural celebrations 

LIFE St. Mary social workers support seniors and their caregivers through advocacy, counseling and caregiver education, and connections with resources like financial or legal guidance. 

LIFE St. Mary dietitians assess each senior’s dietary needs. A snack and a nutritious lunch are provided at the LIFE center and home-delivered meals can be arranged, as necessary. 

Personal care aides come to the senior’s home to assist with daily tasks and activities such as bathing and dressing, meal preparation, laundry or light housekeeping as needed. 

LIFE St. Mary believes spiritual care plays an important role in healing the mind, body, and soul of our participants. Encouraging participants to seek meaning, hope and spiritual connection, chaplains and spiritual companions offer emotional support and resources to participants and family members alike. 

Specialty medical care including dental, podiatry, optometry, and audiology services and supplies is available by our network of medical specialists. Referrals for specialty care must be ap­proved by the Medical Director and interdisciplinary team as being medically necessary. 

There are no out-of-pocket charges if participants qualify for both Medicare and Medicaid or Medicaid only. For participants who don’t qualify for Medicaid, but have Medicare, there is a monthly fee. For participants who do not qualify for Medicaid or Medicare, there is a monthly fee.
