Interdisciplinary Team

LIFE St. Mary provides compassionate, comprehensive care for each participant . This model of care uses an interdisciplinary team (IDT) of health care professionals that work together to treat the whole person with a fully integrated, holistic approach. Every day the IDT meets to discuss the participant's goals and coordinate care, leading to better health outcomes and improved quality of life.

The IDT includes:

Primary Care Provider
Health Center Registered Nurse
Home Care Registered Nurse
Physical Therapist
Occupational Therapist
Recreation Therapist
Social Worker
Dietitian

The day center manager, personal care aide, driver, and chaplain may also consult with the interdisciplinary team to provide complete care for each person we serve.
