Eligibility Requirements

You may qualify for LIFE services if you:

• Are age 55 or older
• Live in one of the service area zip codes
• Are certified by the State as eligible for nursing facility level of care
• Are able to live safely in the community with services provided by LIFE St. Mary

A LIFE St. Mary enrollee may be, but is not required to be, any or all of the following:

• Entitled to Part A
• Enrolled under Part B
• Eligible for Medicaid

LIFE St. Mary will not restrict enrollment based on Medicare or Medicaid eligibility.

LIFE St. Mary services are typically free to seniors who qualify for Medicare and Medicaid or Medicaid only. Participants who do not qualify for Medicaid will be charged a monthly fee and have the option to privately pay.
