Eligibility Requirements

You are eligible to be a participant in Trinity Health PACE Pensacola if, at the time of enrollment, you:

• Are at least 55 years of age
• Meet a nursing facility level of care (LOC) for coverage under the State Medicaid plan
• Live in the Trinity Health PACE Pensacola service area, which includes all of Escambia County and Santa Rosa County.
• Are able to live safely in the community with services provided by TH PACE Pensacola at the time you enroll.

A PACE enrollee may be, but is not required to be, any or all of the following:

• Entitled to Part A,
• Enrolled under Part B,
• Eligible for Medicaid.

TH PACE Pensacola will not restrict enrollment based on Medicare or Medicaid eligibility.

Trinity Health PACE Pensacola services are typically free to seniors who qualify for Medicare and Medicaid or Medicaid only. For participants who do not qualify for Medicaid, there is a flat monthly fee. Participants also have the option to pay for PACE privately.
