The PACE program is an all-inclusive program that combines medical and long-term care services in a community setting for its participants (also known as members).
Seniors qualify for PACE services if:
- They are age 55 or older
- They live in one of the service area zip codes
- They have been certified by the state as eligible for nursing home care
- They are able to live safely in a community setting with services provided by Trinity Health PACE of Pensacola
Trinity Health PACE of Pensacola services are typically free to seniors who qualify for Medicare and Medicaid or Medicaid only. Participants who do not qualify for Medicaid, but have Medicare, will be charged a monthly fee.
