Offering quality, compassionate healthcare to seniors is our goal at Trinity Health PACE. Medical care is provided to participants via care plans designed for their specific needs. Our care plans focus on prevention, diagnosis, treatment, and recovery to help maintain independence and improve seniors' health. The PACE medical team includes primary care physicians, nurse practitioners, registered nurses, and specialists, all experienced in geriatric care. Medical services include:
• Primary medical and specialty care
• Prescribed medications
• Dental and vision care
• Mental health services
• Nursing care
• Durable medical equipment such as wheelchairs, canes, walkers, oxygen, etc.
• Hospital discharge planning and/or nursing home placement.

Studies show that rehabilitation therapy is beneficial to a person's overall health and physical independence. Trinity Health PACE rehabilitation services include physical, occupational, speech, and breathing therapies to help participants recover from illness or injury. Our Certified Rehabilitation Therapists are available to support participants at home or at the PACE day center.
We understand how challenging it is to get to and from appointments when transportation options are limited. Trinity Health PACE offers participants safe, reliable transportation to and from appointments scheduled by PACE at no cost. Our professional staff of drivers and aides support participants as they board and leave the vehicle for a safe and enjoyable ride.
Therapeutic recreation supports and helps maintains seniors' physical and cognitive abilities. Certified Recreational Therapists are a component of our interdisciplinary team that serves to keep participants active and engaged by coordinating recreation-based programs. Our network of Licensed Clinical Social Workers (LCSW) support participants' ongoing needs for social, mental, and financial guidance resources by connecting them to the resources they need to maintain a healthy and independent lifestyle.
Physical activity is just one component of managing a healthy lifestyle—nutrition is another. Our Registered Dietitians outline a nutrition program suitable to the needs of participants in meeting and promoting their health goals. Programs include the treatment of clinical conditions, nutritional education, and supervision of meal plans. We also offer nutritious breakfasts, lunches, and snacks provided at the center. Home-delivered meals are also arranged as needed.
At Trinity Health PACE, we understand the challenges seniors face at home as they grow older. Getting dressed or grooming may become difficult and our compassionate home health aides are available to help participants with personal care.
Our goal as a faith-based ministry is to treat people in mind, body, and spirit. Treating the whole person is more than addressing your physical health. Spiritual care focuses on participants’ purpose and meaning. With the support of chaplains and spiritual companions, we seek to provide peace of mind and improve quality of life.
Specialty medical care such as dental, podiatry, optometry, audiology services and more are available by our network of medical specialists. Referrals for specialty care must be approved by the Medical Director and interdisciplinary team as being necessary. Call us for a full list of specialty care services.
