Saint Francis LIFE (Living Independently For Elders) offers continuous, quality care to the elderly, allowing them the independence of living at home. Saint Francis LIFE is an alternative to nursing homes for those who qualify for nursing home level of care. The LIFE Interdisciplinary Team of experts provides comprehensive care focused on improving participants health and well-being. At the center of Saint Francis LIFE are our LIFE Health Centers where seniors receive clinical and rehabilitation therapy and build relationships with doctors, nurses and fellow participants – all under one roof. As a Program of All-Inclusive Care for the Elderly (PACE), our multi-touch, integrative approach includes services free to those who qualify. Participants must be eligible for Medicaid, Medicaid and Medicare, or able to privately pay.

There are no out-of-pocket charges if participants qualify for both Medicare and Medicaid or Medicaid only. For participants who don’t qualify for Medicaid, but have Medicare, there is a monthly fee. For participants who do not qualify for Medicaid or Medicare, there is a monthly fee.
