Trinity Health PACE Alexandria serves seniors in the community and in our PACE Center. Staying independent and remaining at home is important for seniors, but sometimes living safely at home can be a challenge. Seniors in our program benefit from a wide range of healthcare services provided in our PACE Center or in the home.
Program payment is determined based on Medicare and Medicaid eligibility. Typically, services are free to those who qualify for Medicare and Medicaid, but some participants are required to pay monthly fees if they are over income or asset limits determined by the Centers for Medicare and Medicaid (CMS). Participants that do not have Medicare or Medicaid can privately pay. Participants may be fully and personally responsible for the costs of unauthorized (other than emergency services) or out-of-PACE program agreement services.
Many participants are enrolled in Medicare and qualify for Medicaid. You are likely eligible for this program if, at the time of enrollment, you:
- Are at least 55 years of age.
- Are certified by the State to need nursing facility level of care.
- Live in the TH PACE Alexandria service area.
- Are able to live safely in the community with services provided by Trinity Health PACE Alexandria.
A PACE enrollee may be, but is not required to be, any or all of the following:
- Entitled to Part A,
- Enrolled under Part B,
- Eligible for Medicaid.
Trinity Health PACE Alexandria will not restrict enrollment based on Medicare or Medicaid eligibility.
Trinity Health PACE Alexandria services are typically free to seniors who qualify for Medicare and Medicaid or Medicaid only. There is a monthly charge for participants who do not qualify for Medicaid.
Yes. The focus of every PACE organization is to help you live in the community for as long as possible. To meet this goal, PACE programs focus on preventive care.
Those who are eligible will receive home care services. The Trinity Health PACE Alexandria interdisciplinary team will let you know if your care plan incudes home care services.
Participants at Trinity Health PACE Alexandria must receive care (other than emergency services) from our program's care team and through our network of providers. You will need to see our primary care provider, but your specialists may be a part of our network. Our team will let you know what doctors you will be able to see when you begin the enrollment process. Participants may be fully and personally responsible for the costs of unauthorized (other than emergency services) or out-of-PACE program agreement services.
Participants are picked up by our team of experienced drivers and brought to the Center in the morning. You'll have breakfast with other participants. You may see your doctor or nurse if you have an appointment in the clinic. You might also see your physical therapist or occupational therapist. Participants can enjoy a wide range of activities and groups at the Center. You'll have a healthy lunch, and transportation will bring you back home in the afternoon.
Your benefits under Trinity Health PACE Alexandria can stop if you choose to disenroll from the program (voluntary) or if you no longer meet the conditions of enrollment (involuntary).
If you wish to leave Trinity Health PACE Alexandria, you should talk about it with your social worker who will help you with returning to other Medicaid/Medicare programs for which you are eligible. Your voluntary disenrollment is effective on the first day of the month following the date Trinity Health PACE Alexandria receives your notice of voluntary disenrollment. Trinity Health PACE Alexandria will do everything possible to avoid involuntary disenrollment. We will provide you with reasonable notice before we take any action to disenroll you from our PACE program. Your involuntary disenrollment is effective on the first day of the next month that begins 30 days after the day Trinity Health PACE Alexandria sends you notice of the disenrollment.
