Interdisciplinary Team

Trinity Health PACE provides compassionate, comprehensive care for each participant using a care circle approach. This model of care, called the interdisciplinary team (IDT), treats the whole person with a full-circle view. Every day, the centers’ interdisciplinary team of healthcare professionals discuss the participant's customized care plan. This daily touch base improves health outcomes and quality of life for the participant.

Our IDT includes:

• Primary Care Provider
• Health Center Registered Nurse
• Home Care Registered Nurse
• Physical Therapist
• Occupational Therapist
• Recreation Therapist
• Social Worker
• Dietician

The day center manager, aide, driver, and chaplain may also consult with the interdisciplinary team to ensure holisitic care for each person we serve.
