Our Services
Our full spectrum of services allows seniors to stay healthy and live at home. At the heart of our program is our Saint Joseph PACE Center, including a health clinic with an on-site physician, clinic manager, nurse practitioner, physical and occupational therapy room and three common rooms for socialization, recreational activities, a hot lunch plus morning and afternoon snacks. Services are provided at no cost to the participant and include:

Saint Joseph PACE strives to keep seniors and their family at the center of all medical decisions. All medical care is provided by Saint Joseph PACE’s expert team of primary care providers (PCPs), nurse practitioners and registered nurses. Focused on your complete, preventative and ongoing care, our medical services include:
• Primary and specialty care
• Nursing care
• Medications
• Vision, hearing and dental care
• Foot care
• Mental health services
• Medical equipment and supplies
• Hospitalizations and skilled nursing care
Saint Joseph PACE provides physical, occupational and speech therapies to recover and rebuild your functionality. Our priority is preserving our participants’ independence at Saint Joseph PACE, and we offer different methods and equipment to regain your abilities needed for day-to-day living. Specific goals of rehabilitation therapy vary for each participant, and our team of healthcare providers coordinate to make sure your treatment is tailored to your needs.
Transportation is a covered benefit for Saint Joseph PACE participants. Having the means to safely travel to and from the Saint Joseph PACE Center and specialty medical appointments is an essential part of your care. Our trained drivers will help you make the most of your experience, providing the appropriate accommodations. Each van is equipped with electric lifts for wheel-chair-bound and physically disabled participants.
Though your physical health is important to us, our care plans are focused on treating the whole person- in mind, body and spirit. Our Certified Recreational Therapists seek to create a fun and interactive experience for all participants with hobbies and exercises, intended for your overall wellness. Different activities in our program might include:

• Bingo and trivia
• Holiday and cultural celebrations
Saint Joseph PACE’s full range of care includes social services, offering participants and families the support, resources and opportunities they need to make the best possible decisions. Social workers play a fundamental role on your team of healthcare professionals. Whether it is advocacy, counseling, caregiver education or access to financial or legal guidance resources, you are not alone. Our skilled social workers are in your corner, partnering with you on your path to wellness.
We will assess your nutritional needs and develop meal plans to improve your overall well-being and decrease risk for disease. Evaluating each senior’s dietary needs, our registered dieticians work closely to ensure our participants have access to nourishing food in their home. Nutritious breakfast and lunch meals are provided at the Saint Joseph PACE center. Home-delivered meals can be arranged as needed.
In-home support helps to promote independence for each participant. With personal care aides visiting the senior’s home, you can relax without the stress or challenge of completing regular tasks. Our compassionate aides are here to help, keeping your safety in mind. Whether it is bathing, getting dressed, laundry or light housekeeping, we will assist with day-to-day activities as part of your comprehensive care.
Saint Joseph PACE offers spiritual care to treat the whole person – in mind, body and spirit. Finding meaning, purpose and hope can be part of your spiritual journey. We seek to support, understand and provide resources for you and your family, as you navigate difficult circumstances. With the support of chaplains and spiritual companions, we provide peace of mind and improve quality of life, treating our seniors with a full-circle view.
Specialty medical care including dental, podiatry, optometry, and audiology services and supplies is available by our network of medical specialists. Referrals for specialty care must be approved by the Medical Director and interdisciplinary team as being medically necessary.
There are no out-of-pocket charges if participants qualify for both Medicare and Medicaid or Medicaid only. For participants who don’t qualify for Medicaid, but have Medicare, there is a monthly fee. For participants who do not qualify for Medicaid or Medicare, there is a monthly fee.
