Eligibility Requirements

Your senior may qualify for PACE services if:

• They are age 55 or older

• The live in one of the service area zip codes

• They have been assessed by the state as needing nursing home care

• They are able to live safely at home with services provided by Saint Joseph PACE

Saint Joseph PACE services are typically free to seniors who qualify for Medicare and Medicaid or Medicaid only. For participants who do not qualify for Medicaid, there is a flat monthly fee.
